Consumer Directed Personal Assistance Program (CDPAP)

The Jzanus Consumer Directed Personal Assistance Program (CDPAP) is an alternative to traditional home care.

CDPAP is a Medicaid-funded program that permits the consumer or designated representative to administer and assume responsibility for their own care or the care of their loved one. As a consumer enrolled in this program, you or your designated representative are authorized to hire, train, supervise, and manage the Personal Care Aide caring for you. You are in complete charge of your care and who provides it for you.

How do I become a CDPAP consumer?
If you are a Medicaid recipient that has been approved for home care, and are self-directing – or have a person who is willing and able to be your designated representative – contact the Department of Social Services or the Managed Long Term Plan Case Manager in your county of residence, and request enrollment in the Consumer Directed Personal Assistance Program (CDPAP).

Once you have been approved, contact a Jzanus Home Care Advisor and ask to be connected with a CDPAP Consumer Service Representative that can assist you with enrolling in the Jzanus CDPAP. One of our CDPAP Consumer Service Representatives will walk you through your responsibilities and provide you with the information you need to participate in this unique program.
